Welcome to the Employee Global Wellness Scale (EGWS) - Sample Assessment.

A 20 question (3 minute) research based employee health and wellness questionnaire that measures employee experience and self-perception in the following six key workplace wellness areas: Physical, Emotional, Organizational, Environmental, Personal and Social.

After you complete this survey you will receive a percentile workplace health score for each wellness area along with a global health score that indicates your comprehensive work health.
